Business Banking Membership Documentation List

We're excited that you're ready to open a Business Banking membership. Before starting, it helps to have a few documents ready. Gathering the right information ahead of time can make the process smoother, faster, and easier, so you can get back to business.

Sole Proprietorship

Documents your business MUST provide:

  • Fictitious Business Name Statement (FBNS) or Doing Business as Statement (DBA) if applicable. If spouses own the business, both names must be listed on the document. Business formation requirements vary by state. Please provide the documents required under your state's applicable business laws. Additional documentation may be requested based on your business structure.

OR

  • Business License (sometimes called an Occasional Tax License and must include the first and last name of the owner) – Note: This is only needed if the state does not require an FBNS.

Documents your business MAY need to provide:

  • IRS EIN Letter (Form SS-4 or Form 147C) Note: Sole Proprietors can use their personal Social Security Number (SSN) only if they do not have employees.

Credit Union forms you MUST complete:

  • Sole Proprietorship Membership Agreement
  • Certificate and Resolution

Credit Union forms you MAY need to complete:

  • Deposit Account Resolutions – Note: Only required if the business is owned by spouses and both must sign the form. Multiple forms may be needed if opening more than four (4) accounts.

Limited Liability Company (LLC)

Documents your business must provide:

  • Articles of Organization OR Certificate of Formation
  • IRS EIN Letter (Form SS-4 or Form 147C)

Documents your business MAY need to provide:

  • Operating Agreement – Note: Operating Agreements are not required in some states. Business formation requirements vary by state. Please provide the documents required under your state's applicable business laws. Additional documentation may be requested based on your business structure.
  • Resolution/Authorization – Note: Only required for LLCs with more than one (1) managing member and must be signed by All Partners or the "Managing" Partner if appointed.
  • Foreign Registration Statement, also called Certificate of Authority or Application for Authority to Do Business – Note: Required only if doing business outside of the filing state.

Credit Union forms you MUST complete:

  • Limited Liability Company Membership Agreement
  • Certificate and Resolution
  • Deposit Account Resolutions
    • One form will accommodate four (4) accounts being opened at one time.
    • Signed by all partners or "Managing" partner
  • Certification of Beneficial Ownership Information

Corporation

Documents your business MUST provide:

  • Articles of Incorporation OR Certificate of Corporation
  • Corporate Resolution OR Board Meeting Minutes
    • Must be signed by the Secretary
  • IRS EIN Letter (Form SS-4 or Form 147C)

Documents your business MAY need to provide:

  • Resolution/Authorization
    • Must be signed by the Secretary and only required if the business has one of their own however, our Deposit Account Resolution form is sufficient.
  • Foreign Registration Statement, also called Certificate of Authority or Application for Authority to Do Business – Note: Required only if doing business outside of the filing state.

Credit Union forms you MUST complete:

  • Corporation Membership Agreement
  • Certificate and Resolution
  • Deposit Account Resolutions
    • One form will accommodate four (4) accounts being opened at one time
  • Certification of Beneficial Owner

Partnership

The following applies whether your business operates as a General Partnership (GP), Limited Partnership (LP), or Limited Liability Partnership (LLP).

Documents your business MUST provide:

  • Partnership Agreement depends on the type of partnership:
    • General Partnership
    • Certificate of Limited Partnership
    • Certificate of Registration (for a Limited Liability Partnership)
  • IRS EIN Letter (Form SS-4 or Form 147C)

Documents your business MAY need to provide:

  • Resolution/Authorization – Note: Required if the business has one of their own; however, our Deposit Account Resolution form is sufficient.
  • Foreign Registration Statement, also called Certificate of Authority or Application for Authority to Do Business – Note: Required only if doing business outside of the filing state.

Credit Union forms you MUST complete:

  • Partnership Membership Agreement
  • Certificate and Resolution
  • Deposit Account Resolutions
    • One form will accommodate four (4) accounts being opened at one time
  • Certification of Beneficial Owner

  • Association/Organization Charter OR Bylaws
  • Meeting Minutes (within the last 12 months)
  • IRS EIN Letter (Form SS-4 or Form 147C)

Documents your business MAY need to provide:

  • Resolution/Authorization

Credit Union forms you MUST complete:

  • Unincorporated Association and Organization Membership Agreement
  • Certificate and Resolution
  • Deposit Account Resolutions
    • One form will accommodate four (4) accounts being opened at one time
